Job Description

Our family & locally owned Hospice & Palliative Care is a dedicated healthcare provider with a compassionate focus on supporting patients and their families. We employ a multi-disciplinary approach to hospice care, seeing each patient as an individual with distinctive needs. We are currently seeking a skilled and experienced Director of Nursing to lead our Clackamas, OR office, ensuring the highest standards of patient care and medical compliance. This individual will play a vital role in fostering a positive work environment, implementing medical & process policies, and ensuring we remain the employer of choice in the Clackamas County area for exceptional hospice and palliative care.

Our Director of Nursing (DON) is responsible for the overall direction, oversight, and quality of hospice clinical services. This role ensures care delivery that meets all regulatory standards, supports organizational goals, and aligns with the hospice philosophy of compassionate, patient-centered care.

Key Responsibilities:

  • Provides overall supervision and direction of all direct and indirect patient care delivered by hospice clinical personnel.
  • Guides and supports Clinical Supervisors / Nursing Supervisors to continually improve clinical operations and patient care.
  • Oversees the day-to-day functioning of clinical teams, including assignment of patients, productivity expectations, and census management.
  • Assists in assessing, planning, implementing, and evaluating patient and family/caregiver care when clinical support is required.
  • Interprets operational indicators (census, visit volume, acuity trends) to anticipate staffing needs and address financial impacts.
  • Leads recruitment, hiring, coaching, corrective action, and termination of clinical personnel.
  • Conducts timely performance evaluations and ensures supervisors complete evaluations for their teams.
  • Develops and oversees orientation and onboarding programs for all new clinical staff.
  • Plans and implements in-service education and ongoing training at least quarterly, ensuring competency and regulatory compliance.
  • Builds clinical leadership skills in supervisors, including techniques for evaluating and developing staff.
  • Oversees maintenance and accuracy of clinical records, ensuring compliance with state, federal, Medicare, and Joint Commission standards.
  • Ensures all documentation is accurate, timely, and audit ready.
  • Participates in Quality Assessment & Performance Improvement (QAPI), including quarterly and annual reviews, incident trends, infection control, and clinical quality metrics.
  • Develops, recommends, and enforces organizational clinical policies and procedures.
  • Ensures compliance with all regulatory standards related to licensure, certification, competency, and hospice Conditions of Participation.
  • Assists the Executive Director/Administrator with the preparation and administration of the clinical budget.
  • Monitors staffing utilization, visit patterns, overtime, and productivity expectations.
  • Ensures adequate inventory of clinical supplies, equipment, and medications.
  • Ensures the clinical team communicates consistently, professionally, and compassionately with patients, families, and caregivers.
  • Promotes positive relationships with hospitals, SNFs, providers, and community partners to support appropriate hospice referrals.
  • Maintains strong communication across all departments to support continuity of care and organizational goals.
  • Performs other responsibilities as assigned by the Executive Director/Administrator.

  Required Qualifications:

  • Must have a current Registered Nurse (RN) License in the State of Oregon in good standing for at least seven years.
  • Minimum of seven years of RN experience, including five years in hospice or skilled nursing, is preferred.
  • Experience in a supervisory or leadership role is required.
  • Demonstrated leadership ability with experience supervising clinical and administrative personnel.
  • Strong ability to communicate effectively, both verbally and in writing.
  • Ability to work professionally and tactfully with patients, families, referral sources, and the community.
  • Valid driver's license and dependable, insured vehicle.
  • Knowledgeable in hospice regulations, CMS Conditions of Participation, Joint Commission standards, and general business/management practices.
